A woman arrived at the emergency department with severe nausea, vomiting, and hypoglycemia after a telehealth company prescribed GLP-1 receptor agonists without baseline metabolic assessment or dose titration oversight. This is no longer anecdotal. It's systemic.
GLP-1 receptor agonists—semaglutide, tirzepatide, liraglutide—are potent and specific. They bind to glucagon-like peptide-1 receptors on pancreatic beta cells, dramatically enhancing glucose-dependent insulin secretion. Insulin Level (Fasting): Hyperinsulinemic patients tolerate GLP-1 better than insulin-sensitive patients. This single marker should inform starting dose.
TSH: GLP-1s can unmask or accelerate Hashimoto's thyroiditis. Baseline TSH prevents misattribution of fatigue or weight-loss plateau to the drug.
Lipase & Amylase: Rule out pancreatitis history (relative contraindication to GLP-1).

- If symptomatic hypoglycemia: 15g fast carbohydrate, recheck in 15 minutes
- Consult endocrinology if concurrent insulin or diabetes medications

- Slow, individualized dose titration
- Ongoing glucose and kidney monitoring
- Provider continuity and accountability
A telehealth company that skips these steps is not progressive. It is practicing substandard medicine. Patients arriving at the ER in hypoglycemic crisis are the cost of that negligence.
